使用XDocument根据重复identifier段匹配值移除XML指定Patient元素

问题原因 #

你编写的代码失效核心有两点：

1. 第一段代码中x.Element("identifier")仅会取当前Patient节点下的第一个identifier子节点，且未做空值兼容，存在空引用风险；
2. 第二段代码存在语法错误，("system").Attribute("value")未关联到遍历的identifier节点，逻辑完全不成立。
