PHP表单表格多行提交问题：仅最后一行数据提交的解决办法

问题根源 #

你当前的代码在循环生成表单元素时，所有行的attributeid、csvoption都是同名的普通表单控件，浏览器提交时只会保留最后一行的数值；同时复选框未和对应行的数据关联，无法识别需要提交的目标行。

修改方案 #

1. 前端代码调整 #

将表单控件的名称改为关联数组格式，让每一行的控件与行ID绑定，同时用数组收集选中的行ID：

遍历选中的行ID，根据索引取出对应行的attributeid和csvoption，实现批量提交：
