1. 修复报告创建时的双向关联逻辑

当前你只完成了Report到Patient的单向关联（将Patient的ID存入Report的patient字段），但没有把新建的Report ID添加到对应Patient的reports数组中，导致Patient的reports字段始终为空。

修正后的createReport控制器代码：
